Position Summary
The Case Manager will intake, coordinate, and oversee individualized care plans for pediatric patients, collaborating with healthcare professionals to ensure comprehensive treatment and support for optimal well-being.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
· Providing Case Management services to clients including referral to outside providers as

· Preparing and maintaining records and case files, including documentation, such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.
· Monitoring treatment progress and adjust care plans as necessary.
· Providing education and support to families regarding medical conditions and therapeutic interventions.
· Collaborating with insurance providers to ensure coverage for necessary services.
· Ensuring compliance with regulatory and legal requirements in patient care.
· Collaborating with healthcare professionals to develop comprehensive treatment strategies.
· Conferring with clients to discuss their options and goals so that programs and plans for accessing needed services can be developed.
· Monitoring and record clients' progress to ensure that goals and objectives are met.
· Performing other duties as requested.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Registered Nurse Licensure
- Private Duty Nursing experience
- Trach and Vent experience
- Familiarity with healthcare regulations and compliance.
- Proficiency in medical documentation and record-keeping.
- Knowledge of child development and pediatric medical conditions.
- BLS Certification (Required)
Weekly schedule:
- Monday to Friday 8am-5pm

Compensation: $62,400.00 per year
